We require credit card information on file for no call/no show appointments. Please call the office at 269-857-8310 for secure payment information.

We understand things happen, but we ask that you also understand our groomer is paid on commission and her time is very valuable. We will allow one “oh my gosh I forgot to cancel” but after that, you will be billed 50% of your grooming fee to cover our groomer’s time.

Emergency Policy: In the event of an emergency, we will notify you at the number designated on this form. If we can not reach you, we will contact your emergency contact person.

Illness Policy: If your dog shows signs of parasites, vomiting, diarrhea, contagious skin condition or upper respiratory infection while in our care, we will contact you (or emergency contact if we can’t reach you) for immediate pick up of your pet. You will be charged for any grooming services rendered.

I understand the no call/no show policy stated above.

Mats are a hazard to your dog’s skin and coat. Consent to remove minor mats are included in this agreement and may incur extra charges.

If you want your dog shaved, you understand that stripping your dog it it’s coat does not help your dog stay cool. It actually takes away the dog’s natural insulation (to stay warm and cool) and exposes the dog’s skin to sun burn. It may also affect how your dog’s coat grows back. We will never shave your dog down without your consent.

Payment is due when services are rendered. In the event our groomer finds a need for additional services, we will call you for approval.

Provided all due and diligent care is exercised at all times, I hereby waive and release Dogs Bay and their employees from any and all liability of any nature due to illness and injury.
